Community Case Manager-Care Coordinator-Full-timeCare Coordinator-Community Case ManagerThe Community Case Manager – Care Coordinator, as a member of a collaborative care team, provides intensive case-management services to individuals with serious mental Illness (SMI) in Washoe County who are at risk of or are chronically homeless. The Community Case Manager will provide face-to-face intervention and assist with access to bridge housing and permanent supportive housing. 

Call Center Representative-Full-time. Call Center RepresentativeThe Call Center Representative supports the scheduling of patients providing routing of incoming and outgoing calls, and researching information using available resources.

Clinical Psychologist or Licensed Clinical Social Worker (LCSW)- Full-time: Our Behavioral Health Behavioral Care Provider (BCP) will directly provide evidence based therapeutic interventions to medically, mentally or behaviorally challenged clients of Community Health Alliance. The position will be a part of a multi-disciplinary staff to assess, diagnose, plan and provide treatment for patients. These cases may or may not involve patients with serious mental illness (SMI) or severely emotionally disturbed (SED) clients or clients without an SMI or SED diagnosis but who struggles with managing their physical health, are homeless or under-housed and have chemical dependency issues. The BCP may be asked to facilitate psychotherapeutic and psychoeducational groups to diverse groups of clients.


• Licensed Clinical Social Worker or Clinical Psychologist
• If not licensed in the state of Nevada, then eligibility for licensure in Nevada.
• Be able to pass a background check
• Bilingual preferred.
• Must have reliable transportation to get to and from work
• Willing to obtain needed medical screens and immunizations such as TB test, Hepatitis, etc.
Must keep continuing educational credits up to date and in compliance of law as required by the employee’s governing licensing agency


Advanced Practice Provider-  Full-time: The Advanced Practice Provider is responsible for diagnosing and treating a variety of diseases and injuries as a member of a Care Team in a Family Practice model setting; provides comprehensive, outpatient medical services for individuals and families on a continuing basis; and, may, at certain locations, provide a continuum of care into inpatient and/or residential settings.

We have a full time family medicine physician position available for the right candidate.  Community Health Alliance practices an innovative team care approach to health care allowing the focus of the practice to be on the patient.  As a Federally Qualified Health Center we can provide loan forgiveness through the National Health Service Corporation.  This position offers the opportunity to provide exceptional patient care to those who need it most.


  • Board Certification in Family Medicine

Licensed Practical Nurse (LPN)~ Full-time. LPN Job Description

The LPN provides relevant nursing care in an ambulatory setting. Nursing care is provided under the guidance and supervision of clinical leadership under the Nevada Nurse Practice Act. Responsibilities include telephone and in-person triage; providing lab results; processing home health and DME orders; data collection; CLIA-waived point of care testing; administration of medications, immunizations, and treatments; documentation of care provided; patient education; participating in Code Blue team; and execution of CHA-developed protocols. NO phlebotomy. 

Dental Assistant- Full-time. Dental Assistant.  The Dental Assistant provides assistance to dentists/hygienists in the general treatment of patients in both routine and emergency procedures in areas of diagnostic, preventive, restorative, periodontal, endodontic, pedodontic, orthodontic, oral surgery, and prosthodontic dental care.  This position is Tuesday-Saturday at our Wells Dental Clinic and Mobile Outreach Van.

Medical Front Office Representative-Full-time.  Medical Front Office RepresentativeAt our Neil Rd Health Clinic.  The Medical Front Office Representative performs all front desk activities and ensures completion of necessary paperwork while following all confidentiality and quality management policies.


This organization participates in E-Verify. C.H.A. will provide the Social Security Administration (SSA) and, if necessary, the Department of Homeland Security (DHS), with information from each new employee’s Form I-9 to confirm work authorization.**
