Patient-Centered Medical Home
As part of our ongoing effort to ensure we are providing the highest quality health care in the region, we have sought and have received recognition from the National Committee for Quality Assurance (N.C.Q.A.), a non-profit organization dedicated to improving health care quality, as a Patient-centered Medical Home.
With Community Health Alliance, you are in charge of your health, and we provide you with a team to improve the quality of your health care and shorten the time it takes to get that care. We call this a Patient-Centered Medical Home (P.C.M.H.).
What is a Patient-Centered Medical Home?
A Patient-Centered Medical Home (P.C.M.H.) is a team approach to providing you total, personalized health care. “Patient-centered” is a formal way of saying that you are the focus of your health care. Your team consists of your health care providers, your support system and the leader of the team: YOU. As an active member of your P.C.M.H., you will have a chance to explain the things that are really important to you.
What To Expect
In a P.C.M.H., you and your team will work together to create a health care plan that:
- Is created just for you.
- Connects you with your health care team.
- Is coordinated with other health care providers.
- Supports you every step of the way, (even if that means getting help from other doctors).
