While we treat all people regardless of their ability to pay, and services are provided on a sliding fee scale to the uninsured, we also welcome those on Medicaid, Medicare or private insurance. Patients without insurance are offered a sliding fee scale and are expected to pay their portion of the bill. Community Health Alliance offers the most flexible payment plans available anywhere, and will work with any patient in need of a friendly repayment schedule. We also accept the medical care credit cards.

Yes, all of Community Health Alliance's providers are credentialed by the appropriate licensing body in the State of Nevada.

Community Health Alliance charges for services on a sliding fee scale, which is based on income and family size. In order to qualify for the sliding fee discount, you must bring a number of items in, as described below.

Please bring the following to your eligibility appointment.

  • Government issued identification card of the head of household. (All non-U.S. ID's are accepted.)
  • Proof of address: A current utility bill, rent agreement or lease (see Proof of Address documents) with the name and address of the head of household.
  • Proof of income: The following may be used to prove gross annual income (calculated from the past two months):
    • Paycheck Stubs (two months)
    • Social Security Determination Letter
    • Unemployment Determination Letter

The charge for a medical visit includes the professional fee for our providers, and most of the supplies for that visit. Community Health Alliance has many programs to provide vital health care services, visit the What to Expect page for more information.

Not included in the charge are additional prescription medications, and the charges others will bill for laboratory tests, radiological tests, and fees from other specialists and dentists.

Community Health Alliance could not keep our doors open without patient revenue. As a non-profit we do receive donations from generous individuals, organizations and foundations as well as some federal funding. But the largest source of revenue for Community Health Alliance (70%) is fees paid by patients and their insurance companies for services received.

We ask that you pay for all medical and dental services at the time you receive them. We accept cash, credit cards, debit cards and checks. We also have a payment plan available for those who need to pay for services over a period of time.

  • For our Medicaid and Medicare patients, we are happy to bill the appropriate agency. We are sorry but you will be responsible for any charges not allowed due to ineligibility. If you are deemed ineligible, we will be happy to switch you to a sliding fee scale.
  • For our Sliding Fee Patients, unless you specifically request a customized repayment plan, you remain responsible for your portion of the bill.
  • For our insured patients, we are happy to bill your insurance carrier and you are responsible for any co-payments and deductibles. If for any reason your insurance carrier denies us payment, you remain responsible for charges.

Yes! Community Health Alliance now provides walk-in care at our Wells Ave. Health Center. Come to Community Health Alliance anytime between 7 a.m. - 4 p.m. Monday-Friday and we will get you seen by a medical provider.

A few things you should know…

  • If you feel you are experiencing an emergency: Please inform the front desk and ask for immediate attention. We will have a qualified provider look at you to determine if it is an emergency.
  • If your child has a rash please bring this to the immediate attention of the clinic staff. We will have a qualified provider look at the rash to determine if it is contagious. If it is contagious, we will immediately place you in a separate waiting area.
  • Physicals for school, work and sports are not available on a walk-in basis: We have far too many sick patients to be offering physicals on a walk-in basis. Please schedule your children’s athletic and preschool physicals well in advance of the first week of school or the first day of team practice. If you are here because of an illness and then once in the room ask for a physical, we will treat the illness and ask you to reschedule the physical.
  • Sometimes we must triage: or priority rank our patients by the severity of their illness. From time to time because of the number of patients waiting, the hour of the day, or the severity of an illness we may see a patient with a serious problem first.

I feel like I just walked in and won the lottery…I lost one tooth 2 years ago and I was heartbroken and said I never wanna lose a tooth again.
