Community Health Alliance’s Community Health Workers are trained public health educators improving healthcare delivery with integrated and coordinated services across the continuum of health. Our Community Health Worker (CHW) program is available to assist patients with social drivers of health by connecting patients to resources and assisting with chronic disease management and prevention. Social Drivers of Health (SDOH) are the nonmedical factors that influence health outcomes. They are the conditions in which people are born, grow, work, live, and age.

Community Health Workers address SDOH by utilizing the evidence-based screening tool PRAPARE. The PRAPARE form includes core question domains that screen for personal characteristics, family and home, money and resources, and social and emotional health. Once the PRAPARE form is completed with a patient, the CHW will create community-based organization referrals to any area the patient is in need of assistance with.

CHWs are certified through the Nevada Certification Board and are supervised by a Nevada Medicaid-enrolled physician.

Here is a breakdown of all covered services CHWs can assist you with:

  • Guidance in attaining healthcare services
  • Identify patient’s needs and provide education from preventive health services to chronic disease self-management
  • Information on health and community resources, including making referrals to appropriate healthcare services
  • Connects patients to preventative health services or community services to improve health outcomes
  • Provide education, including but not limited to, medication adherence, tobacco cessation, and nutrition
  • Promote health literacy, including oral health, medication adherence, and consent forms
