We are Community Health Alliance.
We have seven convenient health centers across Reno/Sparks to help you receive quality care. We provide an extensive range of healthcare screening and prevention services, comprehensive primary care for all ages, pharmacy, pediatric dental services, management of chronic diseases, outreach to and services for homeless individuals and families, and behavioral health care. In addition, Community Health Alliance manages a Women, Infants, and Children (WIC) Program. As a Federally Qualified Health Center (FQHC), we also provide community education on health issues and outreach to underserved populations.

Designations and Awards
As part of our ongoing effort to ensure we are providing the highest quality health care in the region, we have sought and have received recognition from the National Committee for Quality Assurance (NCQA), a non-profit organization dedicated to improving health care quality, as a Patient-Centered Medical Home.
A Patient-Centered Medical Home (PCMH) is a team approach to providing you total, personalized health care. “Patient-centered” is a formal way of saying that you are the focus of your health care. Your team consists of your health care providers, your support system and the leader of the team: YOU. As an active member of your PCMH, you will have a chance to explain the things that are really important to you.
You and your health care team will work together to create a health care plan that:
- Is created just for you
- Connects you with your health care team
- Is coordinated with other health care providers
- Supports you every step of the way (even if that means getting help from other doctors)
