Community Health Alliance Nationally Recognized for Its Commitment to Improve Cardiovascular Health

The American Heart Association presents outpatient program achievement awards for proven dedication to ensuring patients have access to best practices that reduce risk of heart disease and stroke.

Community Health Alliance has received three American Heart Association outpatient program achievement awards in recognition for its commitment to reducing the risk of heart disease and stroke by improving high cholesterol, Type 2 diabetes, and blood pressure management. The awards recognize a commitment to following the latest evidence- and science-based care guidelines.

Nearly half of all adults in the U.S. have some form of cardiovascular disease, including heart attack, stroke, or heart failure. High cholesterol, Type 2 diabetes, and uncontrolled high blood pressure are leading risk factors for cardiovascular disease, but with timely diagnosis, research-based treatment and education, these conditions can be managed.

“We are especially proud to announce this recognition during American Heart Month, as it highlights the critical importance of preventing and managing heart disease in our community,” said Community Health Alliance Chief Medical Officer Dr. Travis Walker, MD, MPH, FAAFP. “Our focused efforts on Type 2 diabetes, cholesterol, and high blood pressure management reflect our commitment to putting the latest evidence-based guidelines into action and helping our patients achieve better heart health and overall well-being.”

The outpatient achievement award programs put the unparalleled expertise of the American Heart Association to work for healthcare organizations nationwide, helping ensure the care provided to patients is aligned with the latest evidence- and research-based guidelines. As a participant in the Target: BP, Check. Change. Control. Cholesterol., and Target: Type 2 Diabetes programs, Community Health Alliance was recognized after demonstrating how their organization has committed to improving risk factor management for patients in Washoe County.

In 2025, Community Health Alliance received these achievement awards:

  • Target: BP™ Gold+ recognition jointly presented by the American Heart Association and the American Medical Association for maintaining a 70% or higher blood pressure control rate among adult. Community Health Alliance is the only healthcare organization in Nevada to achieve this distinction in 2025.
  • The American Heart Association’s Target: Type 2 Diabetes℠ Gold Level Recognition for demonstrating impressive HbA1c control in patients with diabetes, along with more than 70% adherence to key cardiovascular-focused clinical measure.
  • The American Heart Association’s Check. Change. Control. Cholesterol™ Gold Level Recognition for ensuring that more than 70% of their at-risk patient population is receiving appropriate statin therapy.
